Helpful Answers for Patients and Families

How can a clinician refer a patient to Dexterity?

Referring a patient is simple: call or message the Dexterity team with a short clinical summary of the case. From there, the team assesses the patient at the bedside, confirms suitability and outlines a rehabilitation plan, then arranges admission with transfer assistance, keeping communication with the referring clinician throughout.

A short clinical summary covering the diagnosis, relevant medical and surgical history, current functional status, and any ongoing medical needs such as feeding tubes, catheters or wound care is enough to begin the assessment. This helps the team evaluate suitability before conducting a bedside assessment and confirming an appropriate rehabilitation plan.

Once a case is shared, Dexterity’s team carries out a bedside assessment and confirms suitability and a care plan promptly, so admission can often be arranged soon after acceptance depending on the patient’s clinical readiness for transfer and bed availability. The referring team can contact Dexterity directly to discuss timing for a specific patient.

Dexterity accepts neurological patients recovering from stroke, brain injury or spinal cord injury who need structured rehabilitation, orthopaedic patients recovering from joint replacement or fracture treatment, and post-acute patients who are medically stable and ready for rehabilitation before going home. Patients who are not yet medically stable or who require acute hospital-level intervention are generally not suitable until that stage has passed; the clinical team can advise on borderline cases during the bedside assessment.

Yes. Dexterity maintains regular progress reports, transparent communication and detailed discharge summaries so that treating clinicians stay connected to their patient’s recovery throughout the rehabilitation stay, and the team follows the referring doctor’s protocols where relevant.

Dexterity provides 24/7 nursing support with a doctor on site or on call overnight, and care is led by senior clinicians who coordinate therapists, nurses and doctors around a single plan for each patient. The team keeps the referring doctor informed and follows their protocols where relevant, with escalation to appropriate medical or hospital-level care arranged if a patient’s condition requires it.
